Trinity Integrated

Privacy Policy

Trinity Integrated Medical — Workshop Website
Effective date: October 7, 2026

This Privacy Policy explains what information Trinity Integrated Medical ("Trinity," "we," "us," or "our") collects through our workshop website at workshop.trinityintegratedmedical.com, how we use it, and the choices you have. It covers our workshop registration pages, sign-up forms, surveys, call-booking pages, and the emails and text messages we send about our workshops.

By using this website or submitting a form, you agree to the practices described in this policy. If you don't agree, please don't use the website or submit your information.

1. Who we are

Trinity Integrated Medical
1629 W Hebron Pkwy, Carrollton, TX 75010
Phone: (972) 478-5538
Website: trinityintegratedmedical.com

2. Your clinical care is covered by a separate notice

This policy covers information you share with us for marketing and workshop purposes, such as registering for a workshop or booking a candidacy call.

If you become a patient, the records of your clinical care (for example, your exam, imaging, treatment notes, and billing) are handled under our separate HIPAA Notice of Privacy Practices, which you will receive at the clinic. If this policy and that notice ever conflict about your clinical records, the Notice of Privacy Practices controls.

3. Information we collect

Information you give us. When you register, fill out a survey, or book a call, we may collect:

  • Your name, email address, mobile phone number, and ZIP code
  • Your answers to survey questions, such as your knee symptoms, how long you've had them, and treatments you've already tried
  • Booking details, such as the date and time you choose for a call and any notes you add
  • Anything else you choose to tell us when you call, text, or email us

Workshop attendance. We use Zoom to host our workshops. We may receive information about whether you registered, whether you attended, and roughly how long you stayed. We use this to send you the right follow-up (for example, a replay link if you missed it).

Information collected automatically. When you visit our pages, we and our service providers may automatically collect technical information, such as your IP address, browser and device type, the pages you visit, the ad or link that brought you here, and actions you take (such as submitting a registration form). See Section 6 for details on cookies and the Meta Pixel.

4. A note about health information on our forms

Some of our survey questions ask about your knee. We use those answers only to contact you about the workshop and about a possible evaluation, and to help Dr. Uselton prepare for any call you book.

Please share only what you're comfortable with. Please don't send detailed medical records, test results, or insurance information through our website forms or by text. If you become a patient, we'll collect that information through our clinic's secure intake process.

5. How we use your information

We use the information we collect to:

  • Register you for a workshop and send your join link, reminders, and the replay
  • Contact you by email, phone, or text (if you've agreed to texts) about the workshop, a candidacy call, or an in-person evaluation
  • Prepare for and hold any call you book
  • Answer your questions and requests
  • Understand which ads and pages are working, and improve our website and workshops
  • Keep records of your communication preferences, including opt-outs
  • Protect our website, prevent misuse, and comply with the law

6. Cookies and the Meta Pixel

Our pages use cookies and similar technologies, including the Meta Pixel (from Meta Platforms, the company behind Facebook and Instagram). These tools help us measure how well our ads work, for example, how many people who saw an ad later registered for a workshop. They may also help us show relevant ads to people who have visited our pages.

The Meta Pixel may collect information such as the pages you visit on this website, the actions you take (such as completing a registration), and technical details about your browser and device. Meta may connect this information with your Meta account and use it under its own data policy, which is available at facebook.com/privacy/policy.

We do not intend to send your survey answers about your knee symptoms or treatments to advertising platforms.

Your choices:

  • You can block or delete cookies in your browser settings. Some pages may not work as well if you do.
  • You can adjust the ads you see on Facebook and Instagram in your Meta account's ad preferences.
  • You can learn more about interest-based advertising and opt out with participating companies at optout.aboutads.info.

Our website does not currently respond to "Do Not Track" browser signals.

7. Text messages (SMS)

Texting is optional. If you check the box agreeing to receive texts, you agree to receive automated and recurring text messages from Trinity Integrated Medical at the number you provided. These may include your workshop join link, reminders, booking confirmations, and follow-up about your inquiry.

  • Consent is not a condition of registering or of any purchase.
  • Message frequency varies.
  • Message and data rates may apply.
  • Reply STOP to opt out at any time. You'll receive one message confirming you've been unsubscribed.
  • Reply HELP for help, or call us at (972) 478-5538.
  • Mobile carriers are not liable for delayed or undelivered messages.

Your mobile information is not sold or shared with third parties or affiliates for their marketing or promotional purposes. Text-messaging opt-in data and consent are not shared with any third parties, except service providers that help us deliver our messages (such as our texting platform and mobile carriers).

8. Emails

You can unsubscribe from our marketing emails at any time using the "unsubscribe" link at the bottom of any email. We may still send you messages you need, such as a confirmation of a booking you made.

9. How we share information

We do not sell your personal information.

We share information only as described here:

  • Service providers that help us run our website and workshops, under agreements that limit how they can use your information. These include our CRM and website platform (GoHighLevel/LeadConnector), our webinar platform (Zoom), our email and texting providers, and our advertising measurement tools (Meta).
  • Legal reasons, when we believe it's required by law, court order, or government request, or to protect the rights, safety, or property of our patients, our staff, the public, or Trinity.
  • Business changes, if Trinity is involved in a merger, sale, or transfer of all or part of its business. If that happens, we'll take reasonable steps to make sure your information continues to be protected.
  • With your permission, in any other situation where you ask us to or agree to it.

If you choose to apply for financing (for example, through Cherry or CareCredit), you'll share your information directly with that company, and its own privacy policy applies.

10. How long we keep information

We keep your information for as long as we need it for the purposes in this policy, such as running the workshop, following up with you, and keeping a record of your communication preferences. We also keep it as long as the law requires. After that, we delete it or remove the details that identify you.

We keep records of opt-out requests (for example, a STOP reply) so we can honor them.

11. Your choices and requests

You can ask us to:

  • See the personal information we have about you from our workshop website
  • Correct information that's wrong
  • Delete your information
  • Stop emails or texts (you can also reply STOP to any text or click "unsubscribe" in any email)

How to make a request:

We may need to confirm your identity before acting on a request. We aim to respond within 45 days. Sometimes we may need to keep certain information, for example, to comply with the law, keep a record of your opt-out, or resolve a dispute. If we can't fully meet your request, we'll tell you why, and you can ask us to reconsider by contacting us the same way.

Requests about your clinical records as a patient are handled under our HIPAA Notice of Privacy Practices.

12. How we protect information

We use reasonable administrative, technical, and physical safeguards to protect your information. No website, email, or text message system is completely secure, though, so we can't promise that information will never be accessed without permission. Please keep this in mind when deciding what to share.

13. Children

Our workshops and website are meant for adults. They are not directed to anyone under 18, and we do not knowingly collect personal information from anyone under 18. If you believe someone under 18 has given us information, please contact us and we'll delete it.

14. Links to other websites and services

Our pages may link to or use services run by others, such as Zoom, calendar apps, Google reviews, and financing companies. We aren't responsible for how those companies handle information. Please review their privacy policies.

15. Governing law

This policy is governed by the laws of the State of Texas and applicable U.S. federal law. Our workshops are intended for people in the United States.

16. Changes to this policy

We may update this policy from time to time. When we do, we'll change the effective date at the top of this page. If we make a significant change, we'll take reasonable steps to let you know, such as posting a notice on our website.
